State Law Addendum<\/h1>\n<p><strong>Last Updated: September 30, 2025<\/strong><\/p>\n<p>This Fitness Center &#8211; State Privacy Law Addendum (\u201cAddendum\u201d) provided by the fitness centers (\u201cFitness Centers\u201d or \u201cwe\u201d or \u201cour\u201d) owned or operated by Premise Health Employer Solutions, LLC (\u201cPremise Health\u201d) supplements the information contained in the <a href=\"https:\/\/premisehealth.com\/privacy-policy-center\/notice-of-privacy-practices-fitness-centers\/\">Fitness Center &#8211; Website Privacy Policy<\/a> (\u201cPrivacy Policy\u201d or \u201cPolicy\u201d) and describes our privacy practices under certain state laws, to the extent such laws apply.<\/p>\n<p>The following state-specific privacy notices are incorporated into this Addendum:<\/p>\n<ul>\n<li><a href=\"https:\/\/www.premisehealth.com\/privacy-policy-center\/fitness-center-connecticut-resident-privacy-notice\/\"><strong>Connecticut Resident Privacy Notice<\/strong><\/a>, which describes rights provided to Connecticut residents under the Connecticut Data Privacy Act (CTDPA), to the extent the CTDPA applies.<\/li>\n<li><strong>Florida Resident Privacy Notice<\/strong>, which describes rights provided to Florida residents under the Florida Digital Bill of Rights (FDBR), to the extent the FDBR applies.<\/li>\n<li><a href=\"https:\/\/www.premisehealth.com\/privacy-policy-center\/fitness-center-nebraska-resident-privacy-notice\/\"><strong>Nebraska Resident Privacy Notice<\/strong><\/a>, which describes rights provided to Nebraska residents under the Nebraska Data Privacy Act (NDPA), to the extent the NDPA applies.<\/li>\n<li><a href=\"https:\/\/www.premisehealth.com\/privacy-policy-center\/fitness-center-nevada-resident-privacy-notice\/\"><strong>Nevada Resident Privacy Notice<\/strong><\/a>, which describes rights that may be provided to Nevada residents under the Nevada Consumer Health Data Privacy Act (NCHDPA), to the extent the NCDPH applies.<\/li>\n<li><a href=\"https:\/\/www.premisehealth.com\/privacy-policy-center\/fitness-center-texas-resident-privacy-notice\/\"><strong>Texas Resident Privacy Notice<\/strong><\/a>, which describes rights provided to Texas residents under the Texas Data Privacy and Security Act (TDPSA), to the extent the TDPSA applies.<\/li>\n<li><a href=\"https:\/\/www.premisehealth.com\/privacy-policy-center\/fitness-center-washington-resident-privacy-notice\/\"><strong>Washington Resident Privacy Notice<\/strong><\/a>, which describes rights that may be provided to Washington residents under the Washington My Health My Data Act (WMHMDA), to the extent the WMHMDA applies.<\/li>\n<\/ul>\n<p>To the extent there is any inconsistency between the terms of the <a href=\"https:\/\/premisehealth.com\/privacy-policy-center\/notice-of-privacy-practices-fitness-centers\/\">Privacy Policy<\/a> and the state-specific privacy notices, the terms of the state-specific privacy notices control.<\/p>\n<p>NOTE:\u00a0 If a state consumer data privacy law currently in effect is <u>not<\/u> listed in this State Law Addendum, we have determined the state law: (i) does <u>not<\/u> apply to Premise Health or the data collected by the Fitness Centers;
