Last Updated: October 22nd<\/sup>, 2025<\/strong><\/p>\n This\u00a0Connecticut Resident Privacy Notice (\u201cConnecticut Notice\u201d) provided by the fitness centers (\u201cFitness Centers\u201d or \u201cwe\u201d or \u201cour\u201d) owned or operated by Premise Health Employer Solutions, LLC (\u201cPremise Health\u201d) supplements the information contained in our Fitness Center – Privacy Policy<\/a> (\u201cPrivacy Policy\u201d or \u201cPolicy\u201d) and our Fitness Center – State Privacy Law Addendum<\/a> (\u201cAddendum\u201d).<\/p>\n If you are a Connecticut resident that visits the Fitness Centers, the information below may apply to you.\u00a0 This Connecticut Notice describes consumer rights under the Connecticut Data Privacy Act (\u201cCTDPA\u201d) and explains how to exercise those rights. The CTDPA protects information that is linked to or is reasonably linkable to an identified or identifiable consumer (\u201cPersonal Data\u201d) and any such Personal Data used to identify a consumer\u2019s physical or mental health condition or diagnosis (\u201cConsumer Health Data\u201d).\u00a0 Personal Data does not<\/u> include information that is de-identified or publicly available.<\/p>\n The CTDPA and this Connecticut Notice do not<\/u> apply to Member health information we maintain in accordance with the Health Insurance Portability and Accountability Act and its implementing regulations (\u201cHIPAA\u201d).\u00a0 That information is governed by the Notice of Privacy Practices<\/a>, which is available at the Privacy & Compliance Policy Center<\/a>.\u00a0 The CTDPA excludes other categories of information that we do not list here.<\/p>\n Any terms defined in the CTDPA have the same meaning when used in this Connecticut Notice.\u00a0 To the extent there is any inconsistency between the Privacy Policy<\/a> and this Connecticut Notice, the terms of this Connecticut Notice apply.<\/p>\n Personal Data We Collect, Use and\/or Disclose <\/strong><\/p>\n The categories of Personal Data we may collect, how we may use that Personal Data and the categories of third parties to whom we may disclose\/share that Personal Data are included in our Privacy Policy<\/a>. We may collect Personal Data from a child but only if the child is eligible for Fitness Center services and with parent permission.\u00a0 We may collect your Consumer Health Data if you provide it to a fitness coach.<\/p>\n As noted in our Privacy Policy<\/a>, we may collect this Personal Data directly from you or our clients (e.g., employers).<\/p>\n Your Right to Know<\/strong><\/p>\n You have the right to know whether we are processing your Personal Data.<\/p>\n Your Right to Access<\/strong><\/p>\n You have the right to access and obtain a copy of the specific pieces of Personal Data we have collected about you, upon verification of your identity.<\/p>\n Your Right to Correct<\/strong><\/p>\n You have the right to request that we correct inaccurate Personal Data that we collected and maintain about you.<\/p>\n Your Right to Request Deletion<\/strong><\/p>\n You have the right to request that we delete the Personal Data that we collected from or obtained about you, subject to certain exceptions under the law.\u00a0 Once we receive and confirm your identity, we will delete (and direct our service providers to delete) your Personal Data from our records, unless an exception applies.<\/p>\n Your Right to Opt Out<\/strong><\/p>\n You have the right to opt out of certain sharing of your Personal Data.<\/strong><\/p>\n Your Right Not to Receive Discriminating Treatment <\/strong><\/p>\n You have the right not to receive discriminatory treatment for exercising any consumer rights described in this Connecticut Notice or the CTDPA.<\/p>\n How to Exercise Your Rights<\/strong><\/p>\n To exercise any of the rights noted above, you may contact us at: Privacy@premisehealth.com<\/a> or via our toll-free phone number (844) 407-7557.<\/p>\n We may ask you to provide additional Personal Data, such as your name and email, so that we can properly identify you in our dataset to track compliance with a request. If you choose not to provide this information, we may not be able to respond to your request if we cannot verify your identity (or your authority to make the request for someone else). In certain circumstances, we may decline a request to exercise the rights described above.<\/p>\n You may designate an authorized agent to exercise your rights on your behalf.<\/p>\n Our Response<\/strong><\/p>\n We will try to respond to a verifiable consumer request within forty-five (45) days of receipt. If we are unable to process your request in such time, we will inform you of the delay in writing by mail or email.\u00a0 Information provided in response to a consumer request will be provided free of charge once during any twelve (12) month period..<\/p>\n If a request is manifestly unfounded, excessive or repetitive, we may charge a reasonable fee to cover the administrative costs of complying with the request or may decline to act on the request.\u00a0 If we determine that the request warrants a fee, we will tell you why we made that decision and provide you with a cost estimate before completing your request.<\/p>\n Your Right to Appeal<\/strong><\/p>\n If we are unable to comply with all or a portion of your request, we will explain the reasons we cannot comply.
