{"id":15537,"date":"2025-11-05T10:00:00","date_gmt":"2025-11-05T16:00:00","guid":{"rendered":"https:\/\/www.premisehealth.com\/?post_type=news&#038;p=15537"},"modified":"2025-11-05T09:23:38","modified_gmt":"2025-11-05T15:23:38","slug":"apha-foundation-awards-premise-health-pharmacy-residents-incentive-grants","status":"publish","type":"news","link":"https:\/\/www.premisehealth.com\/resources\/news\/apha-foundation-awards-premise-health-pharmacy-residents-incentive-grants\/","title":{"rendered":"APhA Foundation Awards Premise Health Pharmacy Residents Incentive Grants\u00a0"},"content":{"rendered":"<section class=\"block block--freeform\">\n<div class=\"container\"><span data-contrast=\"none\">The <\/span><a href=\"https:\/\/www.pharmacist.com\/\" target=\"_blank\" rel=\"noopener\"><b><span data-contrast=\"none\">American Pharmacists Association (APhA) Foundation<\/span><\/b><\/a><span data-contrast=\"none\">, research affiliate of the Washington, D.C., based professional advocacy group, announced Premise Health pharmacy residents Dimple Patel and Dani Warren as recipients of its Incentive Grants for Practitioner Innovation in Pharmaceutical Care.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">Since 1993, the APhA Foundation has awarded Incentive Grants to pharmacists, students, and community residents looking to implement or support existing patient care services within their pharmacy practices, with over $500,000 awarded to more than 600 pharmacy-based projects to date.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">Grants are awarded for research projects across a range of focus areas, including innovative payment models, social determinants of health, behavioral health, the role of pharmacists in primary care, and substance use disorder.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-15538\" src=\"https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/D.Patel_Headshot-683x1024.jpg\" alt=\"\" width=\"168\" height=\"252\" srcset=\"https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/D.Patel_Headshot-683x1024.jpg 683w, https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/D.Patel_Headshot-200x300.jpg 200w, https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/D.Patel_Headshot-768x1152.jpg 768w, https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/D.Patel_Headshot-1024x1536.jpg 1024w, https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/D.Patel_Headshot-1365x2048.jpg 1365w, https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/D.Patel_Headshot.jpg 1825w\" sizes=\"(max-width: 168px) 100vw, 168px\" \/>A first-time recipient, <strong>Dimple Patel<\/strong> is a PGY2 pharmacy resident in Greer, South Carolina. She was awarded the grant to further her <\/span><span data-contrast=\"auto\">assessment of Premise providers\u2019 knowledge, perceptions, and readiness to implement pharmacist-led Collaborative Practice Agreements (CPAs) for chronic disease management.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">\u201cI\u2019m incredibly grateful for this recognition and support from the APhA Foundation,\u201d said Patel. \u201cThis project represents an exciting opportunity to strengthen provider-pharmacist collaboration at Premise and advance our shared mission of improving patient outcomes through team-based care.\u201d<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<hr \/>\n<p><span data-contrast=\"auto\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-15539\" src=\"https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/Dani_Warren_headshot-774x1024.jpg\" alt=\"\" width=\"168\" height=\"222\" srcset=\"https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/Dani_Warren_headshot-774x1024.jpg 774w, https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/Dani_Warren_headshot-227x300.jpg 227w, https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/Dani_Warren_headshot-768x1016.jpg 768w, https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/Dani_Warren_headshot-1162x1536.jpg 1162w, https:\/\/www.premisehealth.com\/wp-content\/uploads\/2025\/11\/Dani_Warren_headshot-1549x2048.jpg 1549w\" sizes=\"(max-width: 168px) 100vw, 168px\" \/><strong>Dani Warren<\/strong>, a Premise PGY2 pharmacy resident in Carrollton, Georgia, was awarded the grant for a second time. This year, her proposal focused on evaluating clinical outcomes for members receiving primary care who also enrolled in a pharmacist-led condition management program through Premise&#8217;s team-based care model<\/span><span data-contrast=\"none\">.<\/span><span data-contrast=\"auto\">\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">\u201cI am grateful for the opportunity to be recognized for my research a second time by the APhA Foundation,\u201d said Warren. \u201cThis project means so much to me as a <\/span><span data-contrast=\"none\">pharmacist, and I hope the results are able to highlight the positive impact Premise pharmacists are having when working alongside our members&#8217; primary care providers.\u201d<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"none\">Patel and Warren will join other recipients next March to celebrate their recognition at the <\/span><a href=\"https:\/\/aphameeting.pharmacist.com\/\" target=\"_blank\" rel=\"noopener\"><b><span data-contrast=\"none\">2026 APhA Annual Meeting<\/span><\/b><\/a><span data-contrast=\"none\"> in Los Angeles, California.
